Depression can come from external situations, such as relationship or job loss, or internal states, often experienced as chronic depression. I try to first of all help the client stop blaming him- or herself for this state. Then I tend to focus on small shifts in the session. For example, if a client starts out being excited about something and then shifts to a flat affect, I will comment on that and together we will try to understand what happend right before the shift. I believe that paying attention to these small and often subtle shifts, right in the session, can help to alter old patterns.
